Q. How do I obtain a Motor Vehicle Fire Report?

A. Motor Vehicle fire reports can be obtained at the Bureau of Fire Prevention,
47 Lowell Street, Peabody.

Our hours of operation are the same as City Hall.
Monday, Tuesday & Wednesday 8:30a.m. to 4:00p.m.
Thursday, 8:30a.m. to 7:00p.m.
Friday, 8:30a.m. to 12:30p.m.

Q. What is the request process?

A. The request must be made in person. The report can be picked up at the Fire Prevention Office. Forms FP33-C and FP33-D must be completed at the Fire Prevention Bureau Office prior to any report being issued. They cannot be handed out to the vehicle's owner, insurance company or agents to be made out at a later date. The forms must be completed at that time in the presence of the Fire Official.

What do you need from me?

A. Fire Prevention needs the date, address, and approximate time of day of the building fire. Proper identification of the requesting party is required.

How Long Will it Take?

A. Allow 5-10 days for data entry and processing. For in-person requests, when information is available, thirty minutes to two hours.

What else should I know?

A. All requests are $5.00. Checks are made out to the City of Peabody.
No mail requests can be made for motor vehicle fires.
Forms FP33-C and Form FP33-D must also be completed. All questions on the form MUST be answered.
If there was no fire department response, only Form FP33-C has to be made out.
