is the request process?
A. The request can be made by phone, mail, or in person. The report can be mailed or picked up at the Fire Prevention Office. |

do you need from me?
A. Fire Prevention needs the date, address, and approximate time of day of the building fire. Proper identification of the requesting party is required. |

How Long Will it Take? A. Allow 5-10 days for data entry and processing. For in-person requests, when information is available, thirty minutes to two hours. |

All requests are $5.00. Checks are made out to the City of Peabody. |
