- Meet
all Requirements of 527 CMR 13.
- List
of last three (3) jobs where you have blasted.
- Double
Matting required on all non-quarry shots.
- Warning
Signals according to 527 CMR 13.
- GEO
Tech on site during blasting operations for Monitoring Blasts.
- Blast
Design, and Blast Analysis submitted to GEO Tech chosen by blasting
company for Approval. Letter of Confirmation submitted to Fire Department.
Current blast design and analysis to be kept on file at the Peabody
Fire Prevention Bureau.
- The
Certified/Licensed Technician will properly fill in and sign the form
as required by the New England Chapter Society of Explosives Engineers
and the State Fire Marshal.
- Hours
of Blasting are 9:30 a.m. until 2:30 p.m.
- No
Blasting on Saturdays, Sundays or Legal Holidays.
- Pre
Blast Surveys in accordance with 527 CMR 13.
- Fire
Dept. Chief or Designee to physically walk site prior to blasting operations
to ensure
250-ft. rule is adhered to.
- Alert
all surrounding Neighbors in blast zone prior to blasting.
- Blasting
Complaints shall be Responded to within thirty (30) days of Complaint
by the Insurance Adjuster.
- Logs
and Seismograph Readings returned DAILY to Fire Prevention. Specify
EXACT location of Seismograph and Blast. Provide the distance from Seismograph
to nearest Blast Hole. (G.P.S. Coordinates are acceptable just for the
location of the Blast.)
- Seismographs
to be kept in proper working order and Certified according to 527 CMR
13 and Mfg.s Specifications.
- Certificate
of Insurance kept on file at Fire Prevention Office with the City of
Peabody as Certificate Holder.
- Blaster
to notify Peabody Fire Alarm Office, 978-531-3444, fifteen (15) minutes
prior to shot.
- Daily
shot reports submitted to the Fire Prevention Office, 47 Lowell St.,
or Fax report to
978-977-0828. All fax reports to be followed by hard copies.
